Welcome to Celebrating Life Community Health Center - Kristina Sergeyeva Family Health Center, and CLCHC Family Health Center. Founded in 2018, CLCHC preserves the legacy of Kristina Sergeyeva who passed away unexpectedly at the young age of 24. CLCHC is HRSA designated Federally Qualified Health Center (FQHC) look-alike health center located in Mission Viejo, CA, with service to the surrounding community. FQHCs, such as CLCHC, are safety net providers that primarily engage in providing medical, dental, behavioral health, and social services in their outpatient clinic in Mission Viejo, CA. The primary focus of the FQHC program is to enhance the provision of primary care services in medically underserved communities.
